Please contact our Guidance Services at: 905-826-1195 Ext. 411. If approved by a V.P., an appointment is scheduled through the Guidance Office.
For former Streetsville students who wish to re-register, please see a Vice-Principal for approval.
PROCEDURES FOR NEW STREETSVILLE STUDENTS WISHING TO REGISTER:
Documents Required for Registration: Please note that the following documents are required at time of the registration appointment. If documents are incomplete or missing your appointment will be postponed to a later date, or put on hold until completed. Our telephone number is: 826-1195 Ext. 411
Proof of Address in Peel:
Property Tax Assessment Bill, or Rental (Formal) Lease, or one of these documents are acceptable as proof. Utillity bills i.e. cable, gas, electric or telephone. We cannot accept driver’s license, or letters written by owner, tenant, etc.
Proof of Birth
Birth Certificate, or Canadian Passport, or Citizenship Card, or IMM Form 1000, or Admission Letter from School Services
Proof of Age:
Cdn Birth Certificate, or Immigration document, or Passport
Proof of Custody(where applicable):
Living with custodial parent
Living with guardian - court order
Immunization Records:
Yellow card not required if you are coming from another school in the PEEL region
Peel Health Form
Record from elsewhere
Academic /School Records:
Updated student transcript from previous/current school, or copies of report cards from grade 9 on, or student status sheet (current), credit counselling summary
* If student is new to the country as a landed immigrant, he/she is required to contact the ESL Reception Centre (905-279-6000) prior to making an appointment with the home school. Once the assessment is done and an appointment is made, please bring Passport, Landed Immigration Form, Proof of Address, Immunization Records and School transcripts (translated into English) to the registration appointment . If a student is coming here on a Student Visa or family Work Permit, LETTER OF AUTHORIZATION
from the Peel District School Board must be accompanied with the above documents. |
